There is probably a lack of nerves underneath the epithelial tissue of the elbow that allow you to feel the pain of squeezing. However, the epithelial tissue contains nerves that make you feel the sharp pain.

That's just a guess though.

i think the skin is just baggy when you bend your elbow so you can not feel it but using you fingernail is pushing the skin against the bone and putting pressure so you will feel it.
